A 24 hour notice is required for cancelling an appointment. If 24 hours notice is not provided, a late cancellation fee of $150 dollars will be charged. If you are late to your appointment by 10 minutes or more, a no show fee of $150 dollars will be charged. A total of 3 no shows/late cancellations in a calendar year will result in discharge from practice.

New patient intake forms must be completed 48 hours prior to the scheduled appointment. If these are not completed, the appointment will be cancelled. All relevant medical records must be sent 48 hours prior to scheduled appointment.
